- ✓ Photocopy of two (2) valid government issued IDs
- ✓ Tax Identification Number (TIN)
- ✓ Two (2) recent 2x2 ID picture
- ✓ Associate Membership Application Form (Properly filled-up)
Steps for Associate Membership
FILL-UP MEMBERSHIP APPLICATION FORM
- Copy of (2) valid Identification Cards (TIN ID and other government-issued IDs)
For Onsite PMES
- Visit Office from Monday to Friday
- Time: 8:00 AM to 5:00 PM
- Open for Walkin Applicants
For Online PMES
- An official invitation will be sent to applicant’s email address. Authorized SFaC Credit Cooperative personnel will issue an electronic copy (e-copy) of PMES Certificate as proof of attendance and completion to applicant’s registered email account.
For Onsite Application
- Visit office to apply or Download and Fill-up Membership Application Form
For Online Application
- Click: Membership Application Form
For Onsite Application
- Finish PMES and get your Certificate of Participation
- Fill-up and Submit your application form together with your PMES Certificate to SFaC Authorized Officer
For Online Application
- Online Validation of Membership Forms after Online PMES.

- Get your Passbook and other proof of membership
For Online Application
- Pay the required FEE (Membership, Shares, Savings, Life Saving, Passbook) to the Official SFaC Credit Cooperative Accounts
- Submit proof of payment to support@sfaccc.com
